Here you will find access to all of the forms required by our child care programs. The YMCA has always focused on meeting the social, emotional and academic needs of our young learners so they can grow into the changemakers of tomorrow—and today is no different.
The Y’s early learning, child care and out-of-school time offerings are staffed with people who understand the cognitive, physical and social development of kids, the support parents and families need and the importance of maintaining a safe, clean environment for children to learn and thrive. At the Y, infants develop trust and security, preschoolers experience early literacy and learn about their world and school-age kids make friends, learn new skills and get help with their schoolwork. LATCHKEY / SCHOOL AGE CHILD CARE
CACFP FOOD PROGRAM FORM: *Note* Please complete forms whether you anticipate you are eligible or not. These forms help us to reduce food costs for all participants.
ILLNESS POLICY: Information regarding our policies and procedures in dealing with child illnesses.
PAYMENT DRAFT FORM: Permission to draft payment for child care services.
SWIMMING PERMISSION FORM (Preschool): Permission to allow your child to swim during child care program activities.
SWIMMING PERMISSION FORM (School Age): Permission to allow your child to swim during child care program activities.
SWIMMING PERMISSION FORM (School Age/City Pool): Permission to allow your child to swim during child care program activities.
PARK TRIPS PERMISSION FORM (School Age): Permission to allow your child visit the city park on walking trips during our program activities.
TRANSPORTATION PERMISSION FORM: Permission to allow your child to participate in transportation via buses for field trips.
FIRST AID PERMISSION FORM: Permission to allow our trained staff to provide basic first aid to your child in the event they require care.
PICKUP PERMISSION FORM: Permission to allow family members and identified persons to pick up your child from our care.
PHOTO RELEASE FORM: Permission to allow photos of your child to be shared within our program, marketing materials, or social media.
